NOT helpful; left a message for one who never called me back. Have taken same form to the courthouse twice to have it lost and nobody knows what to do with it in the first place even though the Judge demanded the signed form.

A small friendly office, full of lawyers. Seriously, most of the attorneys here are genuinely interested. Someone named this Jefferson County Public. The correct name is the Colorado State Public Defender office.
